For someone to be approved for TCM, they must 4 years old and older and receive Medicaid, currently seeing a therapist and have a qualifying diagnosis.

K&G COUNSELING REVIEWS
I can't express enough how invaluable the targeted case management (TCM) service provided by K&G Counseling was for me. Juggling multiple responsibilities and facing mental health challenges, I found myself struggling to navigate the maze of social services and community resources. TCM stepped in as a guiding light, helping me access vital resources like Medicaid, SNAP benefits, and housing assistance. They even supported me in finding a healthcare provider, educational support, and a job opportunity. This service wasn't just a short-term fix; it empowered me to take control of my life, bridging the gap between my needs and the available resources in my community. I'm incredibly grateful for the support and guidance my case manager provided during a critical time in my life.
